Appointment Types & Fee Schedule

Initial Diagnostic Evaluation & Consultation: 90-120 minutes: $500

Follow Up Appointments:

  • 60 minutes: $300

  • 30 minutes: $200

  • 90 minute extended session: $375

  • 120 minute extended session: $425

All fees for service must be paid the day on which services are rendered.  Dr. Csere accepts checks, all major credit card and debit card payments, as well as cash payments for services rendered.

At times you may have additional questions which arise between appointments or you may have work related forms or other medically relevant forms you need filled out by the doctor.  Dr. Csere will return phone calls and complete any necessary letters and forms for no additional fee if not extensive and can be completed in ~10 minutes.  If your question(s) or requested forms to be completed is/are lengthy or complicated and cannot be addressed appropriately within the aforementioned time-frame, an in-person follow up appointment will be required.  If calls are frequent, in-person follow up appointments will need to be made more frequently to discuss your questions or concerns.

Cancellation Policy

Dr. Csere will allow one missed appointment or late cancellation (i.e. an appointment canceled with less than 24 hours notice) with no fee incurred, however for any additional missed appointments or late cancellations a full session fee will be charged.  When there are extenuating circumstances, exceptions may be made at the doctor's discretion.

INSURANCE

Dr. Csere is not a member of any managed care insurance programs at this time, does not process requests for insurance reimbursement, and is therefore considered an "out of network" provider for most PPO plans.  Depending upon your insurance plan, you may be entitled to partial or full reimbursement for any services provided by Dr. Csere.  While you are responsible for discussing any potential reimbursement from your insurance carrier by contacting your insurance carrier directly, Dr. Csere can provide a printed invoice for you to submit to your insurance carrier.  Regardless of your ability to receive any reimbursement from your insurance carrier, full payment for all services rendered is required on the day services are rendered.
